Welcome to Quad C Physical Therapy!  First off thank you for choosing us as your physical therapy providers! We know you and your physician have a choice in your therapy provided and we are pleased to have been part of your selection. It is our goal at Quad C to give you the best overall experience during your time of need. Starting from before you ever enter the door. Here you can find your new patient intake forms and fill them out prior to arriving for treatment. This will make your visit more efficient and time saving for you as a patient. You could also call and have your paperwork emailed to you directly. Your paperwork would then be directly input to your chart for your provider to review prior to your initial appointment. New Patient Intake Forms 

What to Bring...

  • New Patient Paperwork - found online or emailed prior to appointment 

    • ​If you do not choose to fill out paperwork ahead of your appointment, please arrive at least 15 min early for paperwork as this may cut into your treatment time as we have no significant waiting period in our office. 

  • Photo ID and insurance cards ​- call ahead for pre- verification of benefits 
  • Referral - if you have one

What to Expect...

  • You will register with our friendly front office staff. They will assist you with insurance details and paperwork if needed so that you can focus on your rehabilitation. 

  • You will meet with your physical therapist for a thorough evaluation at the time scheduled or very shortly after. 

  • Your therapist will ask questions regarding your past medical history, current pain or recent injury. The more open the line of communication, the better we can assist you. 

  • Your therapist will assess range of motion, strength, coordination and much more depending on what you are joining us for. Your therapist will then create a treatment plan according to findings from your evaluation and discuss this plan with you at your initial appointment. 

  • You will likely perform some type of light initial exercise your first session pending evaluation. Which then you will be set up on an initial home exercise program that will be progressed throughout you time with us to ensure quickest possible recovery.

  • The amount of therapy sessions will be discussed with you by your therapist prior to session ending. You will then schedule your follow up appointments with the front desk. We have a variety of treatment sessions available however "prime times" do tend to fill up quickly. If you have specific times needed please feel free to schedule early out to ensure availability. 

  • Your physical therapist will type up your initial evaluation and send to your referring provider or other medical professional of your choice. 

  • Your therapist is there to answer any and all of your questions, so if you feel you forgot something please fill free to give us a call. 

  • You will also receive a follow up survey text following your initial evaluation. So please let us know how we did or what we can do to improve your time with us! We look forward to meeting you!

What to Wear...

  • Frequent question we get asked is what do I wear? Best answer is you can dress based on your condition. 

  • If you are being treated for your upper body including, neck, upper back or shoulder. Wear clothing that allows access to those areas. 

  • If you are being treated for your lower body, back, hips, or knees. Wear loose clothing or shorts that allows access to those areas. 

  • If you are unable to wear something for ease of access or comfort, no problem we got you covered. We have private treatment areas available and gowns if necessary.
